It could be argued that all of them can be caused by El Niño. Famine can be a consequence of droughts and floods and mosquito breeding would increase in flooded areas and so enhance the spread of malaria. Not all floods and droughts are natural disasters, in fact many floods are essential for the health of river systems and the provision of silt to improve farmland. They also flush out estuaries increasing the fish catch. Droughts are often an inconvenience but not a disaster. It is a badly worded question as the answer could be "all of them", "none of them" or "some of them but different ones at different times".
